The "Clinical Guidelines for Acute Cholangitis and Cholecystitis" and the international version Tokyo guidelines for the management of acute cholangitis and cholecystitis (TG) have been created in Japan as diagnostic criteria for acute cholangitis and are used in many clinical settings. However, it has not been sufficiently studied in clinics. We transported 10 patients suspected of having acute cholangitis to a specialized hospital using the guidelines at our clinic and examined the consistency of the diagnoses. All cases were acute cholangitis, and the diagnosis at the clinic and the diagnosis at the special function hospital agreed (10/10: 100%). The severity of acute cholangitis at the clinic was determined as severe in 2 cases, moderate in 5 cases, and mild in 3 cases. The severity assessment was also all consistent with that of a special function hospital (10/10: 100%). It was suggested that treatment of acute cholangitis using guidelines may be useful in clinics.
