A 63-year-old woman underwent pancreaticoduodenectomy for ampullary cancer. Nine months later, CT revealed increased soft-tissue density in the left side of the superior mesenteric artery. Once superior mesenteric lymph node recurrence was diagnosed, S - 1 administration was started. The lesion temporarily shrank and tumor marker levels decreased; however, CA19 - 9 level increased. Therefore, the regimen was changed to gemcitabine and cisplatin. Although the lesion showed an enlarging tendency, it remained localized, so radiotherapy (50Gy in 25 fractions) was administered. Next, chemotherapy was restarted, CA19 - 9 level normalized, and CT performed at 4 years and 9 months postoperative showed a clinically complete response. She has been followed up without chemotherapy since 5 years postoperative, and no signs of recurrence have been observed as of 5 years and 6 months postoperative. This case demonstrates that radiotherapy can be effective for biliary tract cancer.
