A Case of Conversion Surgery After Long-Term Chemotherapy with Gemcitabine and Cisplatin for Local Recurrence Following Gallbladder Cancer Resection

Abstract

A 64-year-old man who underwent an extended cholecystectomy and partial duodenectomy for gallbladder cancer with liver and duodenal invasion. Postoperative adjuvant chemotherapy with S-1 was initiated. However, 10 months later, CA19-9 level were elevated and a 40mm indistinct mass near the gallbladder bed were detected on CT, indicating local recurrence. Chemotherapy with Gemcitabine and Cisplatin (GC therapy) was selected. After 6 courses, CA19-9 levels normalized, and the recurrent lesion showed significant shrinkage. Conversion surgery was performed after 24 courses of GC therapy. A 20mm mass was palpable in the pancreatic head, and a pancreaticoduodenectomy was performed. The resected specimen showing similar histological features to the previously resected gallbladder cancer, confirming local recurrence. An R0 resection was achieved, and the therapeutic effect was assessed as Grade 1a. The patient completed 6 months of adjuvant GC therapy and has no recurrence for 15 months after surgery.
