Pancreaticoduodenectomy (PD) is the standard treatment for ampullary carcinoma; however, indications for endoscopic papillectomy (EP) remain unclear. We performed EP for total biopsy of a non-exposed protruded ampullary carcinoma filling the papillary lumen. Histologically, it was diagnosed as a papillary carcinoma of the common channel of the ampulla of Vater, with lateral extension. Meanwhile, beneath the invasive area of the common channel of sphincter of Oddi, the tumor transitioned into a moderately to poorly differentiated adenocarcinoma, proliferating with minimal fibrous stromal reaction and forming a distinctive round mass. PD was curative, with no residual tumor. EP may aid in evaluating such atypical invasion patterns and determining the need for further treatment.
