A Case of Type 1 Diabetes Mellitus revealing a Prescise Change in Insulin Requirements during Pregnancy, Perinatal period and Lactation by Continuous Subcutaneous Insulin Infusion (CSII)

Abstract
A 33-year-old woman with type 1 diabetes had been under treatment with of CSII, and successfuly became pregnant by IVF (in vitro fertilization). She kept good glycemic control during pregnancy, perinatal period, and lactation. Basal insulin infusion increased after 20 weeks' gestation, reached maximum insulin requirement at 30 weeks' gestation, decreased just before delivery, increased a little just after delivery, and decreased markedly during lactation. Bolus insulin infusion increased after 20 weeks' gestation, reached maximum insulin requirement just before delivery (double the amount of prepregnancy amount), decreased markedly just after delivery, and reverted to the prepregnancy level during lactation. After the end of lactation, both basal and bolus insulin infusion reverted to the prepregnancy state. We have to take notice of these features of insulin requirement in cases of pregnancy with diabetes.
