Chemical exchange saturation transfer (CEST) has recently emerged as a promising imaging technique. Amide proton transfer imaging, which targets amide protons, is the most widely used CEST imaging method and has been introduced in clinical practice. In this article, we review the principle and diagnostic value of CEST imaging and discuss (1) its diagnostic performance for grading gliomas, (2) its accuracy for predicting glioma molecular subtypes, (3) the differentiation of gliomas from other brain tumor types, and (4) the distinction of tumor recurrence from treatment-related changes.
