A homicide was committed by a person with a mental disorder in Awajishima Hyogo Prefecture on March 9th, 2015. His family was very anxious about his condition and consulted several organizations frequently involved with both the police and public health centers. There were many previous cases similar to this case. In order for such tragic events to not happen again, the first thing we must do is to reflect on the reason why it occurred and why it could not be prevented and to then take appropriate steps to deal with the situation. It is very important to prevent this type of tragic event from occurring. Through significant experience of house calls for 26 years, the author believes that a kind house call by a psychiatrist connected with organizations is an effective solution. It is indeed the work that only a psychiatrist can now legally perform. The psychiatrist can be with patients in times of emergency, reduce their anxieties and also carefully judge the patient’s condition. Even if we perform successful crisis intervention, it is impossible to prevent all events. However, it may be possible to reduce the number of events committed by such patients. Therefore, it is important to create a reasonable system and then carry it out. The author herein describes the service of house calls of Kurashige clinic, and asserts the need of building a rotation system of house calls by volunteered psychiatrists on a voluntary basis.
