A 3 -year-old Chihuahua in her fifth pregnancy presented with severe nonregenerative anemia and was referred from another hospital on day 36 of pregnancy. The dog was followed up without treatment to avoid any adverse effects on the fetus; however, owing to progression of anemia, we administered a whole-blood transfusion, with subsequent, pregnancy termination and bone marrow evaluation under general anesthesia. Bone marrow examination revealed severely hypoplastic erythroid precursor cells, and we diagnosed pure red cell aplasia. Postoperatively, the animal was unresponsive to prednisolone therapy therefore, cyclosporine was added to the regimen. Anemia continued to progress, and we administered a whole-blood transfusion; the animal received four whole-blood transfusions, all of which resulted in a very short red blood cell lifespan. Rapid progression of anemia was observed 12 days after the third transfusion, and we suspected a delayed hemolytic transfusion reaction. Treatment is on going on day 314.
