The diagnostic options for patients with advanced prostate cancer have expanded with the development of next-generation imaging modalities. These include whole-body magnetic resonance imaging (WB-MRI) with diffusion-weighted whole-body imaging with background body signal suppression (DWIBS) and molecular positron emission tomography/computed tomography (PET/CT) using choline or prostate-specific membrane antigen radiotracers. Such modalities improve the detection of whole-body nodal and bony metastases in patients with advanced prostate cancer. A 63-year-old man with castration-resistant prostate cancer (CRPC) underwent stereotactic body radiotherapy (SBRT) targeting two pelvic nodal metastases identified by WB-MRI with DWIBS and subsequently confirmed by 11C-choline PET/CT. This case highlights the potential role of 11C-choline PET/CT and corresponding local treatment in the management of oligoprogressive CRPC.
