A 53-year-old woman was admitted to our hospital with the diagnosis of ileus. We inserted a nasogastric tube to decompress the intestine. Endoscopic examination of the lower digestive tract showed an irregular-shaped ulcer in the terminal ileum causing stenosis, and ring-shaped erosions in the cecum. Since various treatments did not improve the ileus, ileocecal resection was performed. Mycobacterium tuberculosis was detected by acid-fast bacteria culture of the resected specimen, leading to the final diagnosis of primary intestinal tuberculosis. We reviewed previous case reports in Japan of intestinal tuberculosis causing ileus, and analyzed the detection status of Mycobacterium tuberculosis with several methods.
