A 64-year-old man with a 19-year history of ulcerative colitis (UC) (total colitis) in clinical remission was treated with adalimumab. Laboratory examinations at regular outpatient visits revealed increased biliary enzyme levels. Endoscopic, histological, and radiographic examinations indicated a poorly differentiated adenocarcinoma with multiple liver metastases. Retrospective analysis of imaging results revealed mucosal lesions at the appendiceal orifice during the clinical course; however, a definitive histological diagnosis could not be made. Although appendiceal adenocarcinoma associated with UC is rare, if a mucosal lesion is observed in the appendiceal orifice during UC, it should be differentiated from changes secondary to appendiceal carcinoma or chronic appendicitis. If a diagnosis cannot be made, other imaging examinations and surgical interventions for appendiceal carcinoma should be considered.
