Perforation of a biliary plastic stent through oral protrusion of the duodenal papillae is a rare complication of ERCP. We investigated the clinical characteristics and outcomes of nine cases of patients with duodenal papillary perforation, post endoscopic biliary drainage (EBD), at our hospital between 2021 and 2023. The most common primary disease was common bile duct stones (six patients), and all patients underwent EBD with a plastic stent (straight type). In all patients, the stent was removed from the orifice side using grasping forceps when there was duodenal papillary perforation of the biliary stent, and treatment could be continued. None of the patients had any complications associated with the removal of the stent. Although it is possible to safely treat patients with duodenal papillary perforation of the biliary stent, retroperitoneal perforation may cause more severe complications. Therefore, appropriate stent selection and avoidance of long-term implantation are necessary.
