Abstract
In this paper, we will organize the process of transfer of support information and rewrite the individual support plan to the latest information from the viewpoint of information transfer between organizations regarding the ideal way of continuous employment support in supported employment of persons with intellectual disabilities. He pointed out the importance of work. Specifically, as a framework for employment transition support, the information was created based on the concept of (1) PCP (person-centered plan) in creating an individual support plan by showing a “vertical information transfer” model between organizations. That, (2) analysis by functional assessment and PBS (Positive Behavioral Support) are described as a means to find out what the individual can do in the workplace training, and (3) Goals in employment preparation training. The conditions such as the fact that the results so far are described and (4) the positive information obtained in the workplace training for the individual to “do” are specifically described, and that information is sent to the employer side. After the transition, by showing a “horizontal information linkage” model among employees so that it does not become a mere ghost over time, the stages of “inheritance”, “update”, and “evaluation / request” are repeated. He mentioned the importance of accumulating information on what the individual can do in the individual support plan.
