A service model for an ICT system that is a part of value creation systems is an important planning element which influences success or failure to a program. However it is difficult for an owner makes up study since special knowledge is required and the plan ・ operation realistically tends to depend on an IT group enterprise. And troubles arising from this problem are found here and there. This paper proposes a template to make it possible for an owner to study a service model for an ICT system at the scheme stage under this circumstance. Concretely it introduces an ICT operational assessment sheet which was made by combining ITIL ・ CMM and our illustrative example and moreover shows a standard procedure to breakdown from items which owner make up at the scheme stage to each item in the above sheet.
