The Significance of Urine Examination for Bence Jones Proteins in Combination with Ultrastructural Studies in Cutaneous Amyloidosis

Total 21 cases with amyloidosis in the skin were investigated morphologically by light and electron microscopy, and immunologically by double diffusion analyses of the serum and concentrated urine. Skin manifestations of macular amyloidosis (MA) were seen in 17 cases and lichen amyloidosis (LA) was seen in 1 case. Other 3 cases had combinations of MA and LA (maculopapular amyloidosis) in the skin. In 17 of 21 cases, amyloid fibrillar components were found in the dermis without perivascular involvements by electron microscopy. No monoclonal immunoglobulins were found in these 17 cases. In contrast, amyloid fibrils were found around small vessels adjascent to the endothelial cells in the skin in other 4 cases, all of whom were found to excrete Bence Jones proteins (BJP) of kappa type in the urine without serum monoclonal proteins. Amyloid fibrils with perivascular involvement were positive in biopsied materials of the rectal mucosa in 2 cases and the oral mucous membranes in one case. A renal biopsy in a case without perivascular involvement in the skin was negative for amyloid deposits.
