A case of incurable chronic tibial osteomyelitis following open fracture was treated by two staged operation. At first, a considerable number of sequester fragments, all diseased bone and soft tissue, and unhealthy granulation with skin fistula were removed extensively. The tibial bone excised completely through the healthy both proximal and distal part of bone. Then, fixation of this bone deficit about 11cm sized was obtained by use of a Küntcher nail, and bone chips from ilium were replaced between the excised deficit. It goes without saying that the patient was given an efficient and appropriate antibiotics through pre and postoperative terms. The patient was discharged on the ten months after the first surgery.
It seems reasonable to conclude from this case that the surgical therapy of incurable chronic osteomyelitis must be supported that: 1) complete removement of the diseased bone and soft tissue, 2) adequate transplantation, with 3) use of available antibiotics.
