?The coronary artery perfusion is usually performed during aortic valve replacement. It is controversial whether both right and left coronary arteries should be perfused. Some surgeons insist that the perfusion only of left coronary artery is sufficient, insisting the better surgical field.
Authors
perfused experimentally the left coronary artery, checking the pressure changes in the right coronary artery. Our data showed the right coronary artery pressures didn't show any significant changes when the left coronary was perfused under the high pressure. These results suggested that functional intercoronary anastomoses between the right and the left coronaries were not sufficient in normal dog hearts in spite of the anatomical existence of intercoronary anastomoses.
Authors concluded that the both coronary arteries perfusion is recommended during aortic valve replacement, One coronary artery perfusion cannot supply sufficient oxygenated blood to the other side of the heart muscle through intercoronary anastomoses.
