Validation of the prediction criteria for long-term outcomes of pediatric out-of-hospital cardiac arrest (pOHCA)

Abstract

Introduction : We aimed to validate the two criteria that predict 90-days neurological outcomes after pOHCA. Methods : We performed a retrospective analysis of JAAM-OHCA-registry (n=134,951). A total of 319 children were included. Variable of the criteria 1 included prehospital non-return of spontaneous circulation and unwitnessed-arrest ; those of the criteria 2 included unwitnessed-arrest and initial asystole rhythm. Results : Ten patients had favorable neurological outcomes. The specificity and positive-predictive-value were 100% for both of the criteria. The sensitivity and the negative-predictive-value were 55.3% and 6.6% for the criteria 1, and 39.2% and 5.1% for the criteria 2. Conclusions : Regarding clinical application, both of the prediction criteria may not be sufficient.
