In cases of Oehlers class III-b dens invaginatus, the pulp may sometimes show a vital reaction even though there is a periapical lesion derived from the pseudo-root canal. This means that the pulp may often be preserved through root canal therapy of the pseudo-root canal. A case of Oehlers class III-b dens invaginatus of a maxillary left lateral incisor that was treated using a surgical microscope and bioceramic-based sealer is reported. The #12 tooth was diagnosed with class III-b dens invaginatus on the basis of the pre-procedural X-ray, and, since a vital reaction was noted despite the presence of an apical lesion, it was decided to treat the pseudo-root canal. The pseudo-root canal was enlarged and washed under a microscope and then filled by the single cone method, with a bioceramic-based sealer. At one-year follow-up, the symptoms and the lesion had disappeared. In this case, the use of a surgical microscope and a bioceramic-based sealer in the treatment of a pseudo-root canal allowed accurate, minimally invasive treatment.
