Conceptual Forms Model is a mathematical expression of relationship between database and its input/output forms, which enables us to derive possible conceptual forms implied with its targeted data source through mathematical calculation. Recently, we found a more simple expression of the model applied with adjacency matrix. We are able to build a middleware engine for a practical connection of database and form generator. In this study, we depict a prototype of "Conceptual Form Engine."
