Characteristic imaging findings of odontogenic keratocyst in older patients using computed tomography and magnetic resonance imaging compared with young patients

Purpose : Odontogenic keratocysts (OKC) are developmental odontogenic cysts that can be affected by age-related changes in the oral cavity. As OKC often progresses without clinical symptoms, imaging investigations are required for identification and diagnosis. This study aimed to extract computed tomography (CT) and magnetic resonance imaging (MRI) findings from older patients with OKC.

Materials and Methods : We retrospectively reviewed a total of 68 patients (55 males, 13 females, mean age : 50.4±19.7 years, range : 12-86 years) with a confirmed histopathologic diagnosis of OKC who underwent CT and MRI examinations at our hospital. Patients with basal cell nevus syndrome and cases in which image evaluation was difficult owing to artifacts were excluded. Patients with OKC were divided into two groups : those aged ≥65 years and those <65 years. The We compared patient characteristics (sex), and image findings (site, morphology, maximum diameter, CT value and apparent diffusion coefficient value) between the two age groups. Fisher’s exact test was used to examine differences in categorical variables based on OKC status, while a parametric Student’s t-test was used to assess the differences in continuous variables. A P-value <.05 was considered statistically significant.

Results : Significant differences were observed in the site, internal appearance, and the presence or absence of impacted teeth between OKCs in older patients and the control group.

Conclusions : This study elucidated the CT and MRI characteristics of OKC in older patients.
