Abstract : It is difficult to have an effective physical approach to cysts, which often appear on the palatal side of maxillary anterior teeth or to the rear of the distal root of maxillary first molars, and complete extirpation of cysts is often difficult to achieve. Surgical treatment such as decompression therapy is often performed for such cases, but accessory foramen is formed, and the treatment is not ideal for improving QOL of the patient. In the present case, satisfactory treatment of the tooth, in which the dental pulp had died and large cysts appeared after filling had been performed on vital tooth, was accomplished simply by treating the root canal.

 Prior to performing surgical treatment, it is important to make a diagnosis to find out whether it is a bay cyst or not, for which treatment of the infected root canal is effective.
