Surgical techniques of allograft tympanic membrane transplants in 49 patients were classified into 3 groups: Group I: Homograft tympanic membranes (HTM)with or without periost cuff uncovered; Group II: HTM overlayed with canal skin; Group III: HTM overlayed with fascia.
The anatomic success rates for dry and draining ears in each group were respectively: Group I 8/11 (73%) and 1/8 (12.5%), Group II: 9/10 (90%) and 2/3(67%); Group III: 7/8 (87.5%) and 5/9 (55.5%).
Out of 7 cases in group I in which periosteal cuff was utilized 5 broke and 2largely perforated. These results show that infection increases the rate of graft failure and periost cuff should not be used. Postoperative inspection of the healing donor tympanic membrane with fascia (in Group III) confirmed that short process and malleus gradually come out in approximately one year.
