We analysed the stabilograms of subjects standing still, on impulse stimulation. This challenge tests the postural restoration reflex system, which the stabilogram at rest cannot do. We conclude that this method is helpful in the diagnosis of vertigo.

Thirty-five normal subjects were stabirographically studied using a force plate on the rolling platform. The rolling was controled manually between 0 and 0.6 Hz applied at random. The upper limit of power spectrum and phase shif between rolling and body movements were measured.
These parameters were similar to the results of constant rolling of above 0.2 Hz and were influenced by the rolling of above 0.2 Hz in the random rolling.
As the rolling was controled manually, the power spectrum of rolling was not stable and the content of random rolling changed at every measurment. The motor of rolling platform made noise during acceleration and deceleration allowing the subjects know the rythm of rolling.
These problem indicated that this technique was not adequate for the prevention of forecasting behavior.

To evaluate the site of action of immunotherapy on subpopulations of lymphocytes, mononuclear cells were treated with OKT-4 and OKT-8 monoclonal anti-bodies and rabbit serum complement. OKT-8 treated cells showed a proliferative response against D. farinae, though OKT-4 treated cells did not. Supernatants of D. farinaetreated OKT-4 treated cells from three groups (allergic untreated, allergic treated, and controls) had inhibitory activity on D. farinae-induced proliferation of homoglogous lymphocytes. Supernatant of D. farinae-treated OKT-8 treated cells from controls had no effect on lymphocyte proliferative response (LPR), that from untreated patients showed enhancing activity on LPR, however, that from treated patients showed inhibitory activity on LPR. These results suggests that suppressor cells were generated from the OKT-4 cell population through immunotherapy.

Disseminated Intravascular Coagulation (DIC) is sometimes associated with advanced head and neck tumor. Three out of nine patients who developed DIG are presented. Two of the patients were effectively treated with a course of intravenous heparin sodium, but the other was not because of the sudden onset. Chemotherapy seemed to be a trigger of the development of DIC. In advanced cancer, leucocytosis and fever were the first symptoms in the development of DIG.

A series of our twenty-four nasopharyngeal carcinoma cases is reviewed and also we report one special case of them who was first diagnosed as having undifferentiated carcinoma of nasopharynx later he had seminoma, and then malignant lymphoma in the upper gum. This case was considered to be malignant lymphoma from the initial PAP method. The lymphoepithelioma was distinguished from the undifferentiated carcinoma. Four of twenty-four cases were lymphoepithelioma, these tumors grew slowly, and were controlled by irradiation. There were no clinical differences between the non-keratinizing carcinoma and undifferentiated carcinoma. The count of white blood cells was increased in advanced cases, and these patients tended to have disseminated intravascular coagulation.

Thirty-five patients with squamous cell carcinoma of the mobile tongue are reviewed. The 5-year survival rate was 86% in TxNo,54% in TxN
+,80% in Ti,81% in T2 and 50% in T3. The survival rate for TxNo cases was sign ificantly higher than that for TxN
+, cases. Cervical node metastases was more frequently observed in patients treated by radiotherapy than in those undergoing surgery. Surgical therapy of the Ti lesions was found to be very effective, but radiotherapy was less successful in controlling the primary disease. In the patient with T2 lesion, surgical therapy and radiotherapy showed the same results. Frequent follow-ups should be made of the cases for early detection of control failure and salvage by aggressive surgery.

Among 145 cases of laryngeal cancer seen at our department from 1973 to 1985July,109 cases from 1973 to 1983 March, were analyzed statistically. The number in each subdivision was: glottis 68 cases, supraglottis 38 cases, and subglottis 3 cases. The 5-year survival rates in group were 85.4% (glottis),70.4% (supraglottis) and 33.3% (subglottis).

A retrospective analysis was made of 34 cases of hypopharyngeal and/or esophageal carcinoma t-eated from 1972 to 1984. The lesions were found in the priform sinus (17 cases); post cricoid (9), pharyngoesophageal junction (3), posterior wall (2)and unknown (3). According to the UICC classification, there were 3 cases in stage I; 7 in stage II; 11 in stage III and 13 in stage IV. The initial treatment o f the local carcinomas consisted of irradiation in 13, surgery in 21 and no treatment in 3. Three-year and five-year survival rates were 54.4% and 45.6%, respectively.

Surgical techniques of allograft tympanic membrane transplants in 49 patients were classified into 3 groups: Group I: Homograft tympanic membranes (HTM)with or without periost cuff uncovered; Group II: HTM overlayed with canal skin; Group III: HTM overlayed with fascia.
The anatomic success rates for dry and draining ears in each group were respectively: Group I 8/11 (73%) and 1/8 (12.5%), Group II: 9/10 (90%) and 2/3(67%); Group III: 7/8 (87.5%) and 5/9 (55.5%).
Out of 7 cases in group I in which periosteal cuff was utilized 5 broke and 2largely perforated. These results show that infection increases the rate of graft failure and periost cuff should not be used. Postoperative inspection of the healing donor tympanic membrane with fascia (in Group III) confirmed that short process and malleus gradually come out in approximately one year.

Frey syndrome is characterized by profuse sweating and flushing, usually in the area innervated by the auriculotemporal nerve.
A pedicle flap from the sternomastoid muscle was ulitized in selected patients to prevent the occurrence of the Frey syndrome following parotid surgery. We employed this flap on eight cases and all of patient recovered well without Frey syndrome.

We described a method of the reconstruction of the oral vestibulum in the prolabium. The premaxilla is covered by the mucous membranes of the cleft lip margins in the prolabial tissue and reconstructed the oral side mucosal membrane of the prolabium is reconstructed using tissue from the non-cleft side cleft margins. The result is a satisfactory lip structure with a deep oral vestibulum and a good continuous alveolar arch.

To achieve an effective velopharyngeal valval mechanism for patients with abnormalities of speech related to ineffectual velopharyngeal function, we adopted the repush back and levator sling reconstructive operation combined with pharyngeal flap. The cut of the nasal side of the soft palate can easily contact the posterior pharyngeal wall. The raw nasal surface behind the bony end up to the lateral walls completely covered the upper based posterior pharyngeal flap to prevent the wound contraction. This flap does not make the naso-pharyngeal space narrow but mainly covers the raw area. Simaltaneously, the levator sling reconstruction is performed by retropositioning of the levator muscles. Almost all of cases showed lengthened soft palate and good mobility necessary for speech.

Thirty cases of intractable paroxysmal positional vertigo except benign paroxysmal positional vertigo were studied by means of digital subtraction angiography (DSA), which allows real-time digital subtraction of X-ray transmission data from an image intensifier and television system. IVDSA (intravenous DSA) was used to compare blood vessel flow between the right and left vertebral arteries. Abnormal findings were noted in 27 patients, with 10 being affected on the left side,7 on the right, and 10 bilaterally. Twenty-three patients had slightly stenosis, and bending and kinking were observed in 16. DSA results were related to the 6 established types of positional nystagmus. DSA is, therefore, considered to be an important aid for the diagnosis and treatment of positional vertigo.

A case of bilateral serous otitis media followed by bilateral sudden deafness andright facial paresis in a 58-year-old woman is described. When she visited us on October 2, bilateral serous otitis media and sensorineural hearing loss were diagnosedotoscopically and audiometrically. Steroid therapy had been begun 14 weeks previouslywhen she had had left ear fullness which brought her to an otologist, thoughshe could not continue adequate therapy. Ten weeks later (in the beginning of September),she noticed right hearing loss and tinnitus, and was diagnosed as having bilateralsudden deafness. On October 30, right facial paresis developed. The Herpessimplex titer was 4 on October 2, 128 on October 30. Facial weakness recoveredcompletely after 2 weeks, left hearing loss recovered almost completely before the end of December but right hearing loss was marked.

A case of salivary gland choristoma in the middle ear cavity of a 10-year-old girl is reported. The case was associated with a congenital conductive hearing disorder and lipoma in the left temporal region which caused the auricle to shift to the posterior situation. A 6 × 15 × 3 mm mass was found in her left tympanum and ossicular malformations were also detected. After the removal of the mass, her hearing did not improve. The histological findings revealed a salivary gland choristoma.

Implantation of Arenberg Denver inner ear valve is one of the methods of endolymphatic drainage operation for Meniere's disease. A use of this valve was tried for a 38-year-old man with Meniere's disease in our clinic in vain because of obstruction of the tube, which was found at the second operation. The patient had repeated attack of vertigo, necessitating removal of the implant. During his clinical course, inner ear function was measured several times by electrocochleogram technique. We confirmed that volume of the Summating Potential (SP) area greatly increased with the increase of endolymphatic pressure. We, therefore, consider that the volume of the SP area is a better indicator of the presence of endolymphatic hydrops than the generally accepted SP/AP ratio (AP: Action Potential).

A 42-year-old female, who complained of an abnormal sensation in her throat, was found to have a ganglioneuroma. Tumor involved the left superior cervical sympathethic ganglion, and extended into the prevertebral and parapharyngeal space. The ganglioneuroma of the parapharyngeal space is very rare, our case being the fifth reported in Japan.

We describe a 25-year-old woman with carotid body tumor with extension to the infratemporal fossa, pterygopalatine fossa and intracranium. At the first exploration in 1976, the right common carotid artery was ligated, causing left hemiplegia. After two more operations, she was admitted to our clinic, complaining of a painless mass in the right parotid region. Enhanced CT Scan demonstrated a hypervascular tumor in infratemporal fossa, pterygopalatine fossa and intracranium. The tumor of infratemporal fossa and pterygopalatine fossa was extirpated, but at a subsequent operation, the intracranial mass could not be completely removed.

The sleep apnea is characterized by a history of repetitive episodes of upper airway obstruction during sleep. Two children having this syndrome with loud sonorous snoring and cyanosis at night are presented. After adenotonsillectomy, the sleep apnea pattern in these cases were dramatically improved. Physical examination, xerography of the upper airway with nasal airway tube and polysomnographic monitoring are useful for diagnosis of this syndrome and it was concluded that adenotonsillar hypertrophy can play a role in the pathogenesis of obstructive sleep apnea.

The patient was a 46-year-old female who complained of left visual disturbance of 2.5 years duration. Ophthalmologic examination showed a constriction of the ear side, and center of the visual field and visual acuity of 0.02. A CT scan image showed a round shadow in the apex of the orbit and left ethmoid sinus. A tissue specimen was taken and the histopathological diagnosis was a mixed type schwannma. The tumor, which depressed the lamina papyracea laterally, was totally removed through an external route. After surgery, the visual field and visual acuity recovered almost completely.

A 19-year-old woman was referred to us because of persistent fever and progressive dysphagia 5 days prior to admission. She had malaise followed by fever and increasing sore throat for which treatment was unsuccessful. Physical examination showed remarkable swelling and hyperemia of tonsils of Waldeyer ring. Cervical lymphadenopathy was present. The leucocyte count was 9800 with 81 % lymphocyte and many atypical lymphocyte were found. Paul-Bunnell test was × 32. Antibody titer for EB-virus was increased. The diagnosis was infectoius mononucleosis.

We observed abnormal eye movements in a 24-year-old male, who complainedof vertigo. His eye movements were similar to those of opsoclonus or ocular myoclonus,but not complete coincidental. The movements observed were irregular,sometimes non-conjugated, and were synchronized with the movements of his upperlip. In this case, we applied the graphical analysis technique on 16 mm-film of thepatient. It was very useful for evaluation of the synchronization of the movementsof his eyes and upper lip.

Two patients had pansinusitis with nasal symptoms. However,3 patients had no nasal symptoms with normal findings on anterior rhinoscopy, and required further examination of tomography and CT-scan. In 2 of the 3 cases, bony defect or sequester formation in the optic canal was observed. The other 3 cases had no abnormal findings in the optic canal.

The patient was a 20-year-old male who complained of swelling in submandibular area for a year. A large mass extended from the base of the skull to the upper border of the thyroid cartilage. The histopathological diagnosis was pleomorphic adenoma with peroral biopsy. Enucleation of the tumor was done through the a combination of peroral and cervical approaches. Histopathologically, components of the tumor consist of myoepithelia, stroma and a nest of squamous cell with keratinization, mitosis were infrequently encountered, and a part of the tumor capsule was very thin. The tumor seemed to arise from the mucosa of the palate. Salvage operation was done 5 years later, with a transparotid-cervical approach with mandibulotomy in the ramus mandible, which gave a wide field for surgery.

Bilateral recurrent laryngeal nerve paralysis is a condition which often brings dyspnea. Three cases of bilateral recurrent nerve palsy with different etiology are presented. The cause of the first case was aneurysm of the thoracic aorta, that of the second was idiopathic recurrent nerve paralysis and that of the third was thyroid goiter. We carried out the lateral fixation of unilateral arytenoid cartilage on these 3 patients and all of the patients recovered well with tolerable voice.

Two cases of laryngeal foreign bodies are reported. The first case was a 1.3year-old girl, who developed increased stridor and difficulty in breathing after she had eaten a cookie. The tracheostomy was performed in another hospital, and the tracheostoma could not be closed. She was admitted 17 days later, and an X-ray showed a subglottic impaction of a paper clip. The second case was a 1.5-year-old boy. He was admitted with hoarseness of 2 days duration, and an X-ray showed a glottic impaction of a fish vertabra.

Adult laryngeal papilloma, especially keratinized papilloma tends to exhibit malignant transformation. Two patients with papilloma who developed carcinoma later are reported. The first case was a 50-year-old male who underwent laryngoscopic removal in 1981. He returned three years later with multiple tumors of verrucous carcinoma of the larynx, and underwent total laryngectomy. The second case was a 58-year-old male, who had underwent repeated laryngoscopic removal and laser surgery since 1981. Two years later, the tumor recurred and biopsy specimen revealed squamous cell carcinoma. Retrospective histopathological study of the two cases showed that the malignant lesion was present in a part of the initial tissue specimen.
