Limb metastasis of malignant tumors, especially metastasis to a phalange of the hand, are extremely rare. According to our investigation, there have only been a few cases of oral cancer with metastasis to phalanges of the hand. We report a case of squamous cell carcinoma of the tongue that metastasized to a phalange of the hand. A 74-year-old man was admitted to our department with the chief complaint of bleeding from the tongue. Under diagnosis of tongue cancer in the left side (cT4aN2cM0 UICC8th), a subtotal tongue resection, bilateral neck dissection, rectus abdominis flap reconstruction, and tracheotomy were performed. Chemoradiotherapy was performed to prevent postoperative recurrence, however lung metastasis was observed eight months after the operation. An upper lobe pulmonary resection was performed, however bone metastasis was found in the left index finger 11 months after the operation. Amputation of the left index finger was performed. The pathological result was squamous cell carcinoma, with similar histological features to the primary cancer of the tongue. It was diagnosed as tongue cancer metastasis to a phalange of the hand. Left renal metastasis and peritoneal dissemination were observed, and the patient died of the primary disease one year and one month after initial surgery.
