A 59-year-old woman, who was indicated an impacted molar about 30 years ago by a dentist, had noticed some swelling and the spontaneous pain of the left submandibular area. X-ray examination showed an impacted second or third molar, the roots of which exposed vertically from the lower border of the mandible. After antibiotics chemotherapy, an operation from the extra-oral approach was performed under general anesthesia. An incision was made on the skin of the neck beneath the mandible about 7cm in length.
The root of this tooth was out of the lower border of the jaw hanging on neurovascular bundle. The tooth was removed carefully. Postoperative course was not eventful. X-ray examination showed good bone regeneration at 1 year after the operation.
