A rare case of neurilemoma arising in the center of the mandible is reported. The patient was a 19-year-old woman who had no symptoms, except for diffuse mild bony swelling on the buccal side of the right mandibular molar region. Panoramic radiographic examination showed a well demarcated, unilocular radiolucency involving the non-resorbed roots of the mandibular right second premolar and first molar. Microscopic examination revealed Antoni type B tissue dispersed between Antoni type A tissue. Immunohistochemically, the cytoplasm of the tumor cells was stained by S-100 protein and vimentin. The lesion was easily separated from bone and enucleated with a part of the inferior alveolar nerve.
