A rare case of neuroendocrine carcinoma (NEC) of the esophagogastric junction is reported. An 80-year-old man who was aware of black stool was found to have anemia by a family doctor in June 2017. Upper gastrointestinal endoscopy showed a semi-circumferential type 2 lesion at the esophagogastric junction. The histopathological findings of a biopsy specimen showed poorly differentiated adenocarcinoma on hematoxylin and eosin staining. In addition, immunohistological staining was positive for both chromogranin A and synaptophysin. Therefore, NEC with adenocarcinoma was suspected. No distant metastases in other regions were detected on imaging studies. Therefore, proximal gastrectomy with lower esophagectomy was performed in August 2017 in accordance with the Japanese guidelines. The histopathological findings showed Stage II neuroendocrine carcinoma with poorly differentiated adenocarcinoma of the esophagogastric junction. Adjuvant chemotherapy was not given due to the patient's advanced age. Nevertheless, there has been no evidence of recurrence for 36 months after surgical treatment.
