The case of a 65-year-old woman with vaginal discharge of stool due to a sigmoid diverticulo-vaginal fistula is presented. The patient had a history of developmental delay with very low birth weight and of an open hysterectomy for uterine fibroids. Preoperative examination ruled out malignant disease or complicated fistula formation. Laparoscopic sigmoid colectomy and closure of a vesicovaginal fistula were performed. No postoperative complications or symptom relapse was observed. Of colonic diverticular fistulas, vaginal fistulas are rare, limited to cases with a history of uterine surgery, and the feasibility of the laparoscopic approach is not yet clear. In this report, a case of long-term mental retardation and poor performance status who underwent laparoscopic surgery for a sigmoid diverticulo-vaginal fistula and had a good postoperative course is described.
