A study on cytologic criteria for early invading pictures in uterine cervical cancer

Abstract

Cytologic criteria for differential diagnosis of early invasive cancer, cancer in situ and frank invasive cancer were established and their evaluation was discussed. From a viewpoint of invasive pattern of cancer cells into stroma, early invasive cancer was histologically classified into six pictures of M, PF, EB, NG, EC and ES types.
Coincidence rate between the diagnosis based on these criteria and the final diagnosis was found to be high in cancer in situ and frank invasive cancer. In early invading pictures, M and PF types were often diagnosed as cancer in situ, whereas EC and ES types in many cases were diagnosed to be frank invasive cancer. However, EB and NG types were cytologically diagnosed as early invasive cancer in a high incidence.
On the other hand, coincidence rate between the diagnosis obtained by target punch biopsy and the final diagnosis was high in cancer in situ and frank invasive cancer, whereas in early invading pictures, the rate was found to be relatively lower. It was found, however, that in cases of underdiagnosis obtained by initial punch biopsy, the incidence rate of correct diagnosis was improved when these cytologic criteria were applied not only in cancer in situ and frank invasive cancer but also in early invading pictures.
From these results our cytologic criteria will be useful for prospective diagnosis of EB and NG types of early invading pictures as previously reported.
