Meconium peritonitis associated with total colonic aganglionosis in an infant is reported with reviews of literatures. Ultrasonography reviewed retrospectively suggested the presence of ascites due to meconium peritonitis during pregnancy. At laparotomy meconium peritonitis and the perforation of the ileum were found. Double barrel ileostomy, biopsy of the ascending colon, and appendectomy were performed. The diagnosis of total colonic aganglionosis was established by the results of histological study,barium enema, and manometric study. Martin's operation was done at the age of 10 months. This patient achieved fecal continence at the age of 3 years.
