A 7-year-old girl who had complained a recurrent abdominal pain for one year before admission showed abnormal hepatic function. On ultrasonography, the common bile duct was found remarkably dilated during the painful period, while not so during the painless period. Endoscopic retrograde cholangiopancreatography demonstrated a cylindrical dilatation of common bile duct with an anomalous junction of pancreatobiliary ductal system. The operation consited of excision of common bile duct and reconstruction with a hepaticojejunostomy in Roux-Y fashion. Postoperatively she has been free from complaints. Repeated ultrasonographic studies are valuable as diagnostic aids in such cases with recurrent abdominal pain. The cause of abdominal pain in this disease has been discussed.
