A case of Adrenal abscess in a 20-day-old boy was reported. This is a rare disease in a neonates and in our knowledge there were only 14 reported cases in the literature. Clinically, ultrasonography was most helpful exanimation in differential diagnosis and was useful to localize the mass in relation to the surrounding organs and to differentiate between cystic, solid and mixed lesions. Because antibiotics alone is inadequate in treating a walled-off abscess, incision and drainage of the abscess are recommended.
