A solitary liver cyst is rare in childhood and we have found few detailed clinical reports regarding the surgical indications and methods in childhood. This report presents a case with a review of literature. A 35 day-old girl, diagnosed as a giant mesentric or ovarian cyst, underwent surgery. The cyst was identified broadly attached to the inferior part of both lobes of the liver. Then unroofing was performed, as this was thought to reduce the srugical stress which would occur with total cystectomy. She has done well since surgery, without recurrence. Unroofing is a good procedure for small infants with this type of a cyst to decrease surgical stress.
