We report a case of biliary atresia associated with ectopic cystic biliary tissue and pancreatic tissue in the duodenal wall. A 12-day-old girl was referred to our hospital for the investigation of a cyst located between the hepatic hilum and the pancreas, which had been detected prenatally. Duodenal duplication was suspected on ultrasonography, and elective surgery was scheduled. She developed jaundice, and off-white stool was noticed on day 99 of admission. We suspected biliary atresia on the basis of an additional examination. Laparotomy was performed on day 106. We diagnosed her as having biliary atresia IIIb1v and performed portoenterostomy. As a 20 mm cystic structure was detected in the anterior wall of the duodenum, we resected it at the same time. The pathological findings revealed the cystic structure to be composed of ectopic biliary tissue and ectopic pancreatic tissue. The postoperative course was uneventful except for postoperative peritonitis related to bowel perforation. Her jaundice improved after this operation.
