Mesenchymal hamartomas of the liver are rare benign tumors that occur frequently in infants aged less than 2 years. Even though a complete resection of the tumor is the purpose of surgery, it is often difficult to perform if the tumor is large or if it occupies the bilateral lobes. Herein, we report a case of infantile mesenchymal hamartoma of the liver, which occupied the S4 and S8 segments. The patient was found to have a multilocular cystic lesion of the liver, 6.7 × 4.8 cm in size, owing to hepatomegaly at the age of 1 year and 7 months. Tumor enucleation was performed safely after three courses of intracystic sclerotherapy with OK-432. The tumor size was reduced to 3.7 × 3.4 cm before operation. A complete tumor enucleation was performed without complications at the age of 2 years and 5 months. The patient is currently doing well 18 months after the operation. A survey of the present literature showed that this is the second case of mesenchymal hamartoma of the liver, which was excised after sclerotherapy. Preoperative sclerotherapy with OK-432 was useful in reducing the tumor size even in the mesenchymal hamartoma of the liver; thus, it is recommended as an effective treatment modality.
