With the ageing of the population in Japan, more and more people wish to spend their final days at home, so it is necessary to provide efficient home care with limited human capital and to maintain the quality of home care. As one means of achieving this, the introduction of a common ICT tool with various advantages for collaboration between multiple professions is being promoted, but some facilities have not introduced such a tool. Therefore, we analyzed the status of information sharing based on questionnaires to medical institutions in districts where common ICT tools have been introduced and interviews with people involved in home care. The findings indicate that the underutilization of ICT tools for information exchange among various professions is attributed to two primary factors: a lack of awareness regarding the necessity of such exchange and concerns about the implementation burden associated with introducing ICT tools.
