【Purpose】To report a case of chronic myelogenous leukemia showing monocular vision loss as the first subjective symptom.
【Case】In X month 2021, a 20-year-old woman with no systemic history visited the emergency department of our hospital with a chief complaint of sudden vision loss in the right eye and underwent detailed ophthalmic examinations. Her initial visual acuity was 0.1 in the right eye and 1.5 in the left eye. She had internal limiting hemorrhages in her right eye and Roth's spots in her left eye. She was admitted to the department of internal medicine at our hospital on the same day and a systemic examination confirmed the diagnosis of chronic myelogenous leukemia.
【Conclusion】Chronic myelogenous leukemia has few subjective symptoms, and it is rare to be diagnosed only with ocular symptoms as the initial symptom. In case of retinal hemorrhages including Roth's spots, leukemia should be considered as a differential diagnosis even if the patient has no systemic history and a thorough systemic examination should be performed as soon as possible.
