Cardiopulmonary bypass (CPB) with extreme hemodilution without donor blood was performed and the alteration of blood, blood chemistry, bleeding amount and respiratory function were studied for those who were on CPB more than 100 minutes. Hematocrit value went down to 19.5±3.0% during CPB which recovered to 33.5±4.5% at the first postoperative day but after 5-6 days, the value decreased to 28.5±5.0%. These secondary decrease was not seen as to serum protein, platelet counts. Postoperative MB-CPK was 23±18mU/ml at the first postoperative day and 13±10mU/ml at the second postoperative day. There were no cases of postoperative myocardial infarction. Kidney function was always revealed normal. Bleeding during operation amounted to 720±200g and postoperative bleeding was very little, that is 187±71ml during the first 10 hours postop. From respiratory studies, lung compliance was relatively high at 0.046±0.006L/cmH2O at 5-7 hours postoperatively. FRC was 1.4±0.3L which was consistent with the 32.6% decrease of preoperative value of 1.9±0.5L. Extubation of endo-tracheal tube was performed at 9.5±2.4 hours postoperatively, when A-aDO2 was determined to be 321±85mmHg. VD/VT was at that time 45±16%.
