A-4. 痛風治療剤および温泉水飲用のOxypurine代謝に及ぼす影響

Average serum oxypurine level in 43 male gouty patients without H. P. P. medication was 0.5mg/100ml. Average daily urinary oxypurine excretion was 22mg. H. P. P. administration showed a marked increase in daily urinary oxypurine excretion and a decrease in urate excretion. Serum oxypurine levels increased during H. P. P administration. Total urinary purine excretion was reduced in the majority of the cases after a transient increase. But the total urinary purine excretion of a juvenile tophaceous gouty patient showed no decrease.
Influences of benzbromarone on purine metabolism of rabbits, in which uric acid biosyntheses were previously inhibited by H. P. P., were studied under drip infusion containing uric acid. After administration of benzbromarone, urinary excretion of total purine metabolites (urate+allantoin) increased gradually and plasma urate level decreased slowly. When benzbromarone was administered to gouty patients, whose renal functions were within normal ranges, it acted mainly as a uricosuric agent However, in the patients having moderately disturbed renal functions, a marked decrease of serum urate was not constantly followed by any increase of urinary urate excretion. This fact suggests that benzbromarone has a hypouricemic action different from other uricosuric agents. Benzbromarone inhibits xanthine oxidase activity in vitro. Administered to a man, benzbromarone did not change plasma and urine oxypurine levels significantly. So it was thought that this drug had not the inhibitory action on xanthine oxidase in vivo. After intake of water fromKusatsu thermal spring, both plasma urate and oxypurine levels lowered, and urinary urate and oxypurine excretions increased on the contrary. So the water seems to have an action to excrete purines. After intake of water from Ikaho or Atami thermal springs, the changes of purine derivatives in plasma and urine had showed no definite tendencies. Thus further investigations on the effects of the water from both thermal springs will be necessary.
