Abstract
Recently, Japanese health care system has embraced the concept of community and home care. In this paper, firstly the significance and issues of in-home medical care are discussed through the reconsideration of the meaning of “home” as a place for caring. Secondly, referring to the health and welfare policy trends in Japan, how the in-home medical care for older clients are developed and transformed in this decade (2006–2015) is revealed. From the critical point of view, the trends have meant to exclude of older people from their home, community and the medicine. A suggestion for the experts to unravel the complex situation shows last.
