We evaluated the results of high tibial osteotomy in patients over 70 years of age who had medial compartment osteoartrosis of the knee. Twenty-seven knees in twenty-four patients were followed: eleven in men and sixteen in women. The average age of the patients at the time of the procedure was 72.3 years (range, 70 to 83 years) . The mean follow-up period was three years and two months (range, one year and four months to five years and three months) . The functional results were evaluated according to the system of the Japanese Orthopaedic Association for osteoarthritic knees score (JOA score) . We routinely used arthroscopy to determine indications for surgery.
Arthroscopic debridement was performed at the time of HTO. The average knee functional scores improved significantly from 49.6 points before the operation to 82.9 points at the time of the follow-up evaluation. The mechanical axis passed through a point 8.9 ± 19.3% of the width of the tibial articular surface from its medial margin preoperatively and 74.2 ± 11.3% at the follow-up, respectively. The standing femorotibial angle was 184.9 ± 5.0° preoperatively and 167.0 ± 3.4° at the follow-up. In these cases, no serious complications such as peroneal palsy and non-union were observed. Thus, this study demonstrated satisfactory clinical results even in those patients over 70 years of age.
