We reviewed 79 patients (89 hips) of congenital dysplastic hips showing severe osteoarthritis which had been surgically treated by shelf acetabuloplasty with a minimum follow-up of five years. Forty-one hips were combined with muscle release and/or muscle transposition (Group A) . Forty-eight hips were combined with partial resection of the acetabular roof and/or femoral osteotomy (Group B) .
The clinical and radiographic results in these two groups were evaluated. The mean value of JOA hip score (hip score of the Japanese Orthopaedic Association) at five years after surgery was 80 points in Group A and 85 points in Group B. A statistically significant difference (p<0.05) was shown for the restoration of joint space at five years between Group A (44%) and Group B (77%) . The survival rate at 13 years in which severe pain in the JOA score was presumed as end point was 79% in Group A and 98% in Group B. Survival rate at 13 years in which a poor grade of joint space was presumed as end point was 37% in Group A and 88% in Group B.
We conclude that as a joint preserving surgery combination of partial resection of the acetab ular roof and/or femoral osteotomy to acetabuloplasty can in surgical management of the severe osteoarthritic hip.
