Objectives: Postoperative shed blood transfusion is a commonly used salvage-method following major surgeries. The aim of this study was to evaluate pen-operative blood loss and efficiency of shed-blood transfusion following total knee arthroplasty (TKA) .
Patients and Methods: A retrospective analysis of 149 patients, 93 patients with osteoarthritis and 56 patients with rheumatoid arthritis who had undergone TKA, was conducted to calculate the mean total blood loss, mean preoperative autologous blood donation, mean shed blood transfusion and incidence of allogeneic blood transfusion. OrthoPAT equipment (Hemonetics® USA) was used for shed blood transfusion.
Results: On average 96ml (range: 30-635) of blood was lost during operation, whereas an average of 669m1 (range: 50-1460) of blood was lost after the operation. The mean volume of preoperative autologous blood donation was 379m1 (range: 0-800) . The mean volume of shed-blood transfusion was 125ml (range: 0-660) and 78.9% of intra- and post-operative blood loss was salvaged by this method. Allogeneic blood transfusion was needed in two cases (1.3%) who had low haemoglobin levels preoperatively. No complications, such as thrombosis, infection, or allergy were observed.
Conclusion: Post-operative blood-loss was the major blood-loss of TKA, which can be compensated by 400m1 of preoperative autologous blood donation and intra- and post-operative shed blood transfusion.
