Abstract
In the diagnosis of the cancer volume of the depth of invasion of the depressed carcinoma by macroscopical finidngs, the analysis of the characteristics of the depressed floor is very important, classed with the form of cancer edge and the hardness of the cancer lesion.
The majority of the depressed carcinoma have the ulceration in the lesion. Prof. Murakami observed the healing of the malignant ulceration, and called the“Malignant cycle”such a phenomenon. By the rise and fall of the ulceration in the cancerous lesion, the depressed floor showed all kinds of morphological change.
Our purpose of this study is the investigation of the relationship between the depth of invasion and morphological status with the ulecration in the cancerous lesion.
The objects were 31 depressed type gastric carcinomas with the ulceration studied in our institute. The comparative study was carried out between macroscopical and microscopical findings.
The resuts were as follows;
1. The cases in mucosal cancer showed the regular granular pattern on the depressed floor.
2. The Borrmann type 2 or 3 showed the rough nodule or hard flat structure on the floor.
3. In the submucosal carcinoma, the case of minimum invasion to submucosal layer, showed regular granule like mucosal carcinoma and the case of mass invasion showed rough nodule like advanced carcinoma.
In the type IIc like advanced carcinoma, according to the cancer volume of the proper muscle layer, the finding of depressed floor changed the granular pattern to a rough surface.
If there is a relationship between the healing course of the malignant ulceration and cancer invasive volume below the submucosal layer, the finding of the depressed floor shows a good indication for diagnosis of the depth of invasion. In the future, we have to carry out more comparative studies between the macroscopical and microscopical findings.
