Abstract

This presentation highlights recent advances and persistent challenges in physiologically based pharmacokinetic (PBPK) modeling for geriatric populations, as investigated by the IQ TALG Geriatric PBPK Working Group. Age-related physiological changes affecting drug absorption, distribution, metabolism, and excretion are reviewed, alongside a comparison of current PBPK platform implementations. Key limitations are discussed, including inadequate representation of gastrointestinal and ADME protein changes in widely used software, and the critical need for robust physiological and pharmacokinetic data to inform age-specific model parameters. Practical applications are illustrated through case studies and survey results, demonstrating the role of PBPK modeling in optimizing drug development for geriatric population. The presentation concludes with recommendations for future research and collaborative efforts to enhance dose optimization and clinical trial design in geriatric populations.
