Methods of Supporting the Transition from Institutions to Independent Living Settings Through Home Care for Persons with Severe Disabilities: Transition Support Actors, Individual Support, and Community Foundations

Abstract

This study elucidates transition support for persons with severe intellectual disabilities from institutions to independent living settings through home care for persons with severe disabilities. The study used action research to examine a project aimed at supporting the transition to community living, implemented by the Council for Independence Support for Persons with Disabilities in Z city. First, the results demonstrated that the role adopted by the council was that of a transition support actor, aiming to realize the values of the Convention on the Rights of Persons with Disabilities. Second, the same support person remained continually involved; the transition to community goods and activities stayed active while respect for institutional lifestyles was maintained. Non-controlling support was provided, and alternatives to structured support were explored. Third, establishing the infrastructure for community living included securing housing and improving the environment through networking, collaboratively negotiating decisions regarding the allocation of home care hours, gradually handing over and sharing non-controlling support, and providing family support during the transition.
