Pilonidal sinus is caused by the insertion of body hair and it is common in the sacrum. There are many reports on the surgical techniques for pilonidal sinus. We selected a transposition flap for flattening of the cleft and employed this method in 18 cases over a period of 6 years. We investigated their epidemiological features and postoperative course. As a result, we found that many patients were male, young and obese, but there was no common occupation as in past reports. The recurrence rate was 9.1%, which was sufficiently low; therefore, this method was considered to be useful. On the other hand, there was a high rate of postoperative breakdown and methods to decrease this rate are needed in the future.
