Cisplatin (CDDP) had shown potentiation of radiation effect. In order to exploit the radiosensitising activities of CDDP, 12 patients with stage II, M and N squamous cell carcinoma of the larynx and the hypopharynx were treated with chemoradiotherapy consisting of 2 Gy given 5 days a week, combined with concurrent administration of low dose CDDP (3-6 mg/m 2/day), at the Department of Head and Neck Surgery, Kanagawa Cancer Center. The primary lesion of the 12 cases are as follows : 8 patients with laryngeal cancer and 4 patients with hypopharyngeal cancer. Eight patients showed complete response (CR) (67%) and the total response rate (CR +partial response (PR)) was 92%. In the cases with stage II and stage III disease, this treatment demonstrated a fairly good response. The CR rate was 83% and the total response rate was 100%.
CR rate of stage IV patients with concurrent combination of radiation and CDDP was 50%, and with radiation only it was 25%. The control rate of primary lesion was much better than that of neck metastases in advanced cases.
Ten patients (83%) showed mucositis and 3 patients showed renal dysfunction. These toxic effects were reversible and all patients could complete radiotherapy.
Concurrent combination of radiation and CDDP seemed to have an excellent effect against laryngeal and pharyngeal cancer and to contribute to laryngeal preservation.
