Integral reform of medicine and nursing care in Japan is ongoing with a target year of 2025, when Japan’s so-called baby-boom generation (born 1947-49) will be 75 years or older. In this study, which focuses on the regional medical vision and the integrated community-based care system, which will be key policies in that reform, we clarified the background behind the “2025 Problem”, which has become a policy issue, the definition of terms, and the policy formation process, and investigated the regional medical vision and the integrated community-based care system. The results revealed three main questions: (1) What is the aim of the regional medical vision?, (2) How is the integrated community-based care system positioned within the regional medical vision?, and (3) How should the relationship between the regional medical vision and the integrated community-based care system be understood?
